Acute and Chronic Disease Management
Harpley Doctors provides acute and chronic disease management from our clinic in Harpley Town Centre, Werribee. Acute problems come on suddenly and need prompt care, while chronic conditions such as diabetes, high blood pressure and asthma need regular review over months and years. This page explains how our GPs handle both, including GP chronic condition management plans and what Medicare covers.
Short-term illness and injury
For a sudden illness or injury, book online or walk in; we are open seven days. A standard consultation runs for up to 15 minutes. All consultations are bulk billed. If you need a blood test, pathology collection by Australian Clinical Labs is on site on weekday and Saturday mornings. For sports and joint injuries, see Sports Medicine. See Patient Info and Fees for fees and practice policies.
If you are seriously unwell, call 000. When the clinic is closed and your problem is urgent, phone 13 SICK (13 7425).
Chronic disease management with a regular GP
Good chronic disease management works best with one GP who knows your history. Our doctors with an interest in long-term conditions include Dr Reda Aboelsaad (diabetes, hypertension, respiratory and kidney disease), Dr Khalida Batool (diabetes, hypertension and heart disease) and Dr Reza Mirfakhraei. Our reminder system prompts you when checks fall due.
GP chronic condition management plans
Since 1 July 2025, a GP chronic condition management plan has replaced the older GP management plans and team care arrangements. According to Services Australia, a plan suits a condition that has lasted, or is likely to last, six months or longer, and it can be reviewed every three months. If you are registered with MyMedicare, your plan and reviews need to be done at the practice where you are registered.
With a plan, Medicare subsidises up to five allied health visits a year, such as a physiotherapist, dietitian, podiatrist or psychologist, and more for Aboriginal and Torres Strait Islander patients. Your GP arranges the referrals.
Care at home and by phone
Phone consultations suit some follow-ups but not anything that needs an examination, and you need at least one face-to-face visit a year to stay eligible. Home visits are available for clinic patients too unwell to attend.
Related care
Long-term conditions often sit alongside other needs. Our GPs also offer pain management, mental health support and regular health checks, and vaccines such as the pneumococcal vaccine for people with risk conditions. To make an appointment, book online or call (03) 8731 1330.
