Medical Certificates and Reports We Provide

Our GPs prepare reports and assessments for many purposes. If you are unsure which kind you need, ask whoever requested it before you book. Common types include:

  • Medical certificates
  • Letters and reports for your specialist
  • Information requested by your employer
  • Reports for legal proceedings
  • Pre-employment medical examinations and job evaluations
  • Taxi and driver’s licence medicals
  • Medical information for WorkCover claims

What a Report Covers

A medical report is written from your medical record and, where needed, an examination. Depending on its purpose, it may cover your diagnosis, relevant history, test results, treatment, and how your condition affects your work or daily activities. Your doctor can only include what is clinically supported, so a report may not say everything you would like it to.

Before Your Appointment

When you book, let reception know that your appointment is for a report and what it is for, so enough time can be set aside with your doctor. Bring any forms you have been asked to have completed. It also helps to bring copies of any relevant letters, test results or scans from other providers.

Your Privacy

Your medical record is a confidential document, and only authorised staff can access it. Relevant information may be shared with doctors and specialists involved in your care, and disclosed where required by law, such as for court orders or subpoenas. If you have any concerns about how your health information is shared, please discuss them with our team before your report is prepared.

Frequently Asked Questions

What if I can’t make my appointment?

Please let reception know as early as you can on (03) 8731 1330. Our cancellation policy is on the Patient Info and Fees page.

Can my report be done over the phone?

Phone consultations are not recommended where a physical examination is needed, so please book an in-person appointment for any medical or assessment that needs one.

Can reception tell me my test results for the report?

No. Results are not given over the phone by reception or nursing staff. Please book an appointment to discuss them with your doctor.

How long will my report take?

It depends on the type of report. Please ask reception when you book.

Do you see WorkCover patients?

Yes. Please inform your employer first and bring your employer’s contact details, so we can verify the claim and arrange payment.

Can I bring an interpreter?

Yes. Tell reception before your visit, and we can request an onsite interpreter in advance or arrange a telephone interpreter.
