Dr Reda Aboelsaad, Dr Khalida Batool and Dr Arezoo Mirzaei all list diabetes management among their interests. Seeing the same GP over time means your results and goals are already known.

The yearly cycle of checks

The NDSS annual cycle of care for diabetes includes:

  • HbA1c (average blood glucose) at least every 6 to 12 months
  • blood pressure at every visit, or at least every 6 months
  • a foot check at least yearly, more often if you are at higher risk
  • an eye check by an optometrist or ophthalmologist at least every 2 years
  • kidney tests every 12 months, plus cholesterol, weight and waist

Blood tests can be collected at our on-site pathology.

A chronic condition management plan

A GP chronic condition management plan sets out your goals and care, and can give access to Medicare-subsidised allied health visits such as a dietitian, podiatrist or diabetes educator. Our Acute and Chronic Condition Management page explains how plans work.
